The hazards of living without a spleen were recognised by the paediatricians in the early 1960's when they focussed attention on the syndrome of fulminant sepsis, often due to pneumococcal infection, occurring in young children within the fIrst two years of splenectomy 1. A child, well at breakfast, might be febrile by lunch, comatose by evening and dead by the following morning. We now recognise that this danger of post-splenectomy sepsis (PSS) extends into adult life and splenectomised patients remain at risk 10, 20 and even 30 years after the operation 2.

Primary pulmonary hypertension is a rare disease of unknown case. Functional hyposplenism is characterized by the appearance of abnormal circulating erythrocytes despite the presence of a spleen and is associated with a variety of disease states. A case of primary pulmonary hypertension associated with functional hyposplenism is described in this report.

Eight patients with inflammatory bowel disease were tested for their ability to mount a serological response to the intravenous particulate antigen phi X174. Seven patients had extensive ulcerative colitis and one patient extensive Crohn's colitis, four having severe hyposplenism and four normal splenic function. The antibody response to phi X174 was grossly impaired in those with hyposplenism.
